Hosting a Five Wishes Party is a fun and easy way to introduce Five Wishes to your friends and family.
Here’s how to do it:
Step 1: Invite all the adult members of your family and any neighbors who are interested. Perhaps each person brings food to be shared later with the group.
Step 2: Have at least one copy of Five Wishes for each member of the group. Have a few extra copies to send to those who can’t attend.
Step 3: A facilitator should educate the group and answer any questions about Five Wishes. Ideally, this person should be someone who can draw out family and friends who may be afraid to ask questions. Prepare by reviewing the Sharing the Gift, a guide to help presenters, and use the Five Wishes Video. Consider the Five Wishes Family Package!
Step 4: Once Five Wishes is completed, signatures should be witnessed or notarized according to the laws of your state. Check your state requirements by visiting FiveWishes.org.
Step 5: Copies of completed Five Wishes should be given to and discussed with the person’s health care agent, power of attorney, doctors, specialists and anyone else the person wishes. Keep the wallet card found in the back of Five Wishes that tells where the original document is located.
